EAU 2018: Minimal Invasive Simple Prostatectomy: Analysis of 207 cases
Dr. Carrion (MD) of the Hospital Universitario Nuestra Senora La Candelaria in Santa Cruz de Tenerife, Canary presents a retrospective analysis of 207 simple prostatectomy procedures performed via laparoscopy.
As a retrospective cohort analysis, the present study reads similarly to a feasibility study. Mean age and PSA was 68.93 years and 5.7 ng/mL, respectively. Mean operative time of the simple prostatectomy was 126 minutes (range 45 to 320 minutes). Post-operatively, peak flow rate improved from a baseline of 8.3 to 21.0 and all patients experienced an improvement in IPSS score (p<0.001). Of note, this cohort is similar to other centers reporting the simple prostatectomy as a feasible means to treat LUTS in BPH patients.
In contrast to the favorable outcomes and comparable patient cohort, however, was a 4.2 day average hospital stay and 7% transfusion rate. Other complications were Febril UTI in 6 patients, hematuria in 15 patients, and urinary retention in 2 patients. In response to critiques from the chairs and audience, Dr. Carrion asserted that these patients were of higher risk for post-operative complication and recurrence and, despite limitations, concludes that a laparoscopic simple prostatectomy is a good treatment alternative for LUTS in patients with BPH. Future prospective studies are recommended to confirm this benefit.
