EAU 2018: What Patients Really Think About Urolithiasis Guidelines: Barriers To Guideline Implementation From The Patients' Perspective

Copenhagen, Denmark (UroToday.com) Recommendations for the management of urolithiasis are published in the 2015 EAU Urolithiasis guidelines (ULG). However, many patient’s beliefs and procedure preference contrast with these guidelines. Dr. S. Sallami and team explored this issue to identify barriers to implementing EAU guidelines.

The authors surveyed 341 patients with newly diagnosed calcium urolithiasis on their opinion and preference for treatment. Before answering, each patient was given an explanation of all the EAU ULG recommended treatments including: medical expulsive therapy (MET), PNL, RIRS, SWL, URS, and open surgery. For this study, patients under 18 years old, pregnant patients, patients with stones under 6 mm, stone in nonfunctioning kidney, urinary malformation, and neurogenic bladder were not included.

Overall, patients preferred URS and open surgery over SWL. The authors found that the male gender and high instruction level were some barriers to EAU ULG acceptance. Age and rural vs urban origin had no influence on patient’s preference.
