(UroToday.com) The 2022 Annual Meeting of the American Urological Association (AUA) was host to The International Bladder Cancer Group (IBCG) AUA Bladder Cancer Forum which featured a brief State-of-the-Art Lecture discussion that followed the presentation by Dr. Arnulf Stenzl regarding the role of transurethral resection of bladder tumor (TURBT). Dr. Psutka concisely outlined the key elements in her practice for an excellent TURBT:
- Attitude: We must not underestimate the importance of this procedure as a diagnostic and therapeutic modality. While procedures for more advanced disease such as a radical cystectomy, be it open or robotic, are more technically complex, the value of a well-performed TURBT has long been established.
- Strategy: Be prepared, organized, have a key strategy to handle complex TURBTs. Many TURBT referrals in academic centers are re-staging TURBTs, thus one has an obligation to maximize the potential of disease free states in this population. Utilizing enhanced imaging with blue light cystoscopy, good control, and excellent technique are essential components of well-thought-out strategy. These principles need to be emphasized to trainees to maximize outcomes and minimize adverse complications, such as an obturator reflex and significant bleeding.
- Checklists: To ensure key elements of a TURBT such as the bimanual exam in females and routine use of blue light at the end of the case to evaluate for residual disease are not forgotten. One must not underestimate the significance of this procedure and ensure these measures are taken to maximize outcomes for our patients.
